My bet, is if the pain is managable, and he won't do much further damage he'll go.
He's not an Olympic caliber thrower.

He didn't participate in any qualifiers, in fact, he hasn't thrown competitively since he left University of Texas.

So, surgery or not, we will not be seeing him in Beijing.
